NETWORK RAIL – CLOSE CALL VIOLATION

 

I have received a report from the lead officer relating to his concerns as to the use of the NR/L3/OPS/045/3.06FA Decision Making Tool Assessment and the NR/L3/OPS/045/3.06FB Post Incident Review Form - his concerns particularly relate to the lack of shared learning following an event. 

 

In connection with this your NEC (National Executive Committee) met on 6th November 2025 to consider this issue, when they adopted the following report:

 

We note the correspondence on file from our representatives.

 

We further note that since the implementation of Modernising Maintenance there has been a worrying trend of weakening industry Rules and Standards.

 

The General Secretary is instructed to write to Network Rail, raising concerns about the use of the NR/L3/OPS/045/3.06FA tool. Specifically, the General Secretary to request data on how frequently this tool has been used, which rules or regulations have been overridden, and the rationale for using the tool in each instance.

 

Any further reports to be placed back before this NEC.
